Your employer is providing you an opportunity to buy valuable life insurance for you and your family. Best of all, it's available to you at affordable group rates, without medical questions and through the convenience of payroll deduction.

 

Who is eligible?

You, as an active, full-time employee, who work 20 or more hours each week.

 

 

  How much insurance can I buy?

 For full-time employees, the coverage is 
 available in $10,000 increments. The 
 maximum total amount of insurance is five
 times your salary not to exceed $300,000
 with Guarantee Issue up to $100,000. 
 Amounts above the Guarantee Issue limit of
 $100,000 will be subject to Satisfactory
 Evidence of Insurability. Your coverage will
 be reduced by 50% at age 70.

  Can I buy coverage on my spouse
  and dependent children?

 Yes, you can buy life insurance for your
 spouse and dependent child(ren) in the
 amount of $10,000. Your spouse can apply
 for additional insurance in increments of
 $5,000 not to exceed 50% of that which the
 employee applies for.  All amounts in excess
 of $10,000 will be subject to Satisfactory
 Evidence of Insurability. *1  Your spouse's
 coverage will terminate when he/she attains
 the age of 70.
 

 Your dependent child(ren) are eligible from
 live birth to under 21 years of age. Children
 who are full-time students, wholly
 dependent upon you for support, are eligible
 up to age 25.

How are premiums paid?

Premiums are paid through the convenience of payroll
Deduction.

If my spouse and I both work for my employer, can we have coverage as an employee and spouse?

No. You cannot be covered both as an employee and spouse.  You should each buy coverage as an employee.

What happens if I become totally disabled?

If you become disabled before age 60 and remain so for nine consecutive months, you can keep your coverage in force to your Social Security Normal Retirement Age without paying premiums for as long as your total disability continues and you provide proof of disability as required.

 If you become disabled after age 60,
 coverage continues at no cost for one
 year or until your Social Security Normal
 Retirement Age, whichever is first.
 

  What if I am not active at work
  on the date coverage is to begin?


  our coverage will begin when you
 resume active; full-time work.
 

  Will my dependents' coverage
  start on the date of issue?

 Yes, unless your spouse or dependent
 child(ren) is hospital-confined or
 disabled, and unable to engage in most of
 the normal activities of a person in good
 health of the same age and sex. Coverage
 on disabled dependents will be delayed
 until they are no longer disabled.

Will I be able to receive any portion of my benefit before I die if I am diagnosed with a terminal illness?

 

If you become terminally ill, you can receive 25% of the death benefit up to a maximum of $25,000. In most states you can collect 25% of your death benefit (up to $50,000) if your life expectance in six months or less.  For complete details please see your certificate of insurance.

 

If I leave my employer can I keep my Voluntary Life coverage?

If your employment ends, you and your spouse (under age 70) and your eligible children (under the limiting age) may, within 45 days, elect to convert your group

 term insurance to an individual group
 term policy without evidence of good
 health by paying an annual premium to
 the company. At age 70 (for you and
 your spouse) and the limiting age (for
 dependent children) coverage may be
 converted, within 45 days, to an
 individual whole life policy.

 These options are not available if
 coverage ends because of non-payment
 of premium or if the participating
 employer moves this plan to another
 carrier.

  Exclusion

 Death from suicide is excluded for two
 years from the insured's effective date of
 coverage (may vary by state law).

 If selected, provides $10,000 in
 coverage for your spouse and all eligible
 dependent children. Your spouse's
 coverage cannot exceed 50% of your

 Accidental Death & Dismemberment

 Optional AD&D insurance is also
 available to all those electing life coverage.
